The Gwinnett Retirement Services Department is here to assist with questions or concerns you may have as an Active Retiree of Gwinnett County Public Schools. Our Retirement Staff is available to provide support and guidance during your retirement years. We have two convenient locations at 437 Old Peachtree Road in Suwanee and 53 Gwinnett Drive in Lawrenceville. Normal business hours for the Gwinnett Retirement Services Department are 7:00 a.m. to 4:00 p.m. Please contact our office at 678-301-6267(A-K) or 678-301-6269(L-Z) to speak to one of our Retirement Assistants.

Health Insurance Reminder

Attaining Age 65 Once Retired? You should enroll in only Medicare Part A and Part B three months before your 65th birthday. To complete this process, please visit your local Social Security office.

You should also contact State Health Benefit Plan (SHBP) a minimum of 30 days prior to you and/or your covered dependent(s) turning age 65 to elect a Medicare Advantage Plan. Failure to do so will result in increased monthly premiums.

Disabled Once Retired?  Please visit your local Social Security office.

Forms

Active Retirees are encouraged to keep personal information such as name, email address, mailing address, phone number, and direct deposit data current. If you need to make changes to your GRS related information, fill out the applicable form and mail to Gwinnett Retirement Services Department at 437 Old Peachtree Road NW, Suwanee, GA 30024 or fax to GRS at 678-301-6275.

GRS Data Change Form
This form allows you to inform GRS of a name, email address, mailing address, phone number, or other contact information change.

Retiree Direct Deposit Authorization Form
This form allows you to have your retirement benefits deposited directly into your checking, savings, or money market account for the appropriate pay period.

*Retiree Optional Benefit Change Form
This form allows retirees to cancel/decrease optional benefits coverage.
*Please contact the GRS office at 678.301.6267(A-K) or 678.301.6269(L-Z) for a Retiree Optional Benefit Change Form.

Retiree Optional Benefit Qualifying Event Change Form
This form allows retirees to make changes within 31 days of a 'Qualifying Event'.

Tax Withholding Certificates for Pension/Annuity Payments:

Federal W-4P
The current year Federal W-4P tax form.
The completed form should be returned to the Gwinnett Retirement Services Department.

Georgia G-4P
The current year State of Georgia G-4P tax form.
The completed form should be returned to the Gwinnett Retirement Services Department.

Life Insurance Designation of Beneficiary: (Please note: Term Life Insurance terminates at age 70)

MetLife Basic and Supplemental Life Insurance Designation of Beneficiary
This form allows you to designate or make changes to your basic and supplemental life insurance.

MetLife Basic $15,000 Life Insurance Designation of Beneficiary
This form allows you to designate or make changes to your basic $15,000 life insurance.

MetLife Supplemental Life Insurance Designation of Beneficiary
This form allows you to designate or make changes to your supplemental life insurance.
